Click to Close Window CDA's procedure is as follows: If you are located in our area and have decided on CDA as your DPO, indicate this on your application. In order for CDA to service you, you must select a general practice dentist from our list of providers. When our administrative office receives your application, a copy will be sent to the selected dentist. Upon your effective date, you may call the dentist's office to begin to schedule appointments. CDA does the rest! Whenever you see a dentist, you will sign a claim form in the office. You are responsible for the co-pays as outlined in your coverage book. All of our dentist's offices also have copies of the coverage as outlined by the state. You will notice on the providers list that other specialist dentist's are listed. These specialists are provided for your dental service by CDA. However, you will be seen by these dentists only by referral from your General Practice Dentist.
